State Board of Dentistry
Philip T. Siegel, DDS, Chairperson
State Tax Compliance
Within the next few weeks Dentistry Board licensees who owe Pennsylvania state taxes or have delinquent tax filings will be receiving letters from the Board directing the licensee to contact the Pennsylvania Department of Revenue to resolve the issue. Since the Board does not have licensees' tax records, please do not contact the Board about this issue. Instead, please contact the assigned Department of Revenue representative listed in the letter with any questions, concerns or to make arrangements to resolve any outstanding tax issues. If you are unable to reach the Department of Revenue representative at the telephone number listed in the letter, you may try reaching that person using 717-787-3911 as a alternative telephone number for the Pennsylvania Department of Revenue.
